Setting up an EHR for a sports performance facility

The moment a physician, PT, or athletic trainer works your floor, you're keeping medical records. Here's how to set that up without slowing training down.

Performance facilities grow into healthcare sideways. You start as a training business — coaches, programming, memberships — and then you add a sports-medicine physician, a physical therapist doing return-to-play work, an athletic trainer running rehab, maybe IV therapy or recovery services. Somewhere in that growth, part of your operation became a medical practice, and gym software can't carry it: training apps don't keep clinical records, won't sign a BAA, and were never meant to hold an athlete's medical history.

Draw the line between training data and medical records

Programming, loads, and performance metrics can live in your training stack. But anything clinical — injury evaluations, treatment notes, medication and health history, rehab progressions run by a licensed provider — belongs in a real chart, in a HIPAA-compliant system with a signed BAA. Minors make this sharper: youth athletes mean parental consents and health histories you're obligated to protect.

Put onboarding in the patient portal so it happens before day one:

  • Health history and PAR-Q-style screening, so red flags surface before training starts.
  • Consents and waivers — including parental consent for minors — signed electronically and stored in the record.
  • Baseline movement screens and injury history charted at intake, so return-to-play decisions later have a starting point.

Charting that keeps pace with the floor

Nobody on a performance floor is sitting down for ten minutes of typing between sessions. Set up short templates per encounter type — injury eval, treatment/rehab session, follow-up screen — that a provider can complete on a tablet in a minute or two. The goal is a real clinical record of every provider touch without pulling anyone off the floor.

Where it pays off is return-to-play: a documented chain from injury to evaluation to progression to clearance. When a parent, coach, or team physician asks why an athlete is or isn't cleared, the chart is your answer.

Memberships and packages, not claims

  4. Day 4: Set up memberships, blocks, and add-on billing; migrate any existing records — that's the vendor's job.
  5. Day 5: Dry-run an athlete journey: intake, screen, session note, invoice, rebook.
